- Candidates who have successfully completed a B.Sc. (Nursing) course with the integrated curriculum of Certificate in Community Health for Nurses (CCHN) or a Post Basic B.Sc. (Nursing) course with the integrated curriculum of Certificate in Community Health for Nurses (CCHN) from an Indian Nursing Council/ State Nursing Council recognized Institute or University from the academic year 2020 and onward are eligible to apply.

- The successful candidate will be appointed as a Community Health Officer on a contractual basis at sub-center level HWCs and will be paid Rs 20,500 (Twenty Thousand Five Hundred) per month honoraria plus up to Rs 15,000 (Fifteen Thousand) per month Performance-based incentive (PBI) at the time of posting as a CHO at an SC-HWC in the allocated district, subject to a GoI guideline.
- At the time of joining as CHO, the candidate must submit a Surety Bond of Rs. 2.50 Lakh (Two Lakh and Fifty Thousand) on Non-Judicial Stamp Paper of Rs. 100/- to serve for a minimum of three years at Sub Health Centre level HWC in Uttar Pradesh.
